Our Direct Primary Care membership offers personalized, accessible healthcare with priority scheduling, direct provider access, and included in-office testing — all at a transparent monthly rate.

What's Included
  • One visit every 6 weeks (routine follow-up, chronic care management, or sick/acute visit)
  • In-person or telehealth visits when clinically appropriate
  • Same or next business day availability for sick concerns
  • Same week availability for routine care management
  • In-office testing included for sick visits: Urinalysis, Rapid Strep, COVID, and Influenza testing

  • $100 enrollment fee (waived if signing up same day as consultation)
  • Billing begins the month following enrollment
  • 6-month commitment required · Auto-renews every 6 months
  • Cancellation required at least 1 month prior to renewal
  • Monthly autopay required · 3% processing fee on credit cards · No fee with debit, HSA, or FSA
  • Not included: send-out labs, in-office IV therapy, injections, procedures, in-office medications (10% member discount applies)
  • This membership is not insurance and does not replace health insurance coverage
