Prevent & Sustain Medicine is an independently owned, cash-based practice in Richmond, Kentucky. We were founded on a simple belief: too many people are living exhausted, dismissed, and without real answers — while being told everything is "fine."
We offer longer appointments, deeper lab testing, and individualized plans built around the root cause of how you feel — not just the symptom in front of us.
Our providers bring years of clinical experience across some of the most demanding environments in medicine — including ICU and critical care, nephrology, urology, emergency medicine, and urgent care. That depth of multi-specialty training is what allows us to see connections that routine primary care often misses.
In a traditional practice, visits are often limited to 15 minutes, labs are restricted to what insurance will cover, and care is reactive — meaning you're treated after something goes wrong. We work differently:
Longer visits. Your initial consultation is a full 60 minutes dedicated entirely to you.
Deeper testing. Advanced panels that uncover what standard labs miss.
Root-cause focus. We look at hormones, metabolic health, vascular function, and more as an interconnected system.
Collaborative decisions. You are in the driver's seat. Nothing is handed down to you.

Direct Primary Care
Direct Primary Care is a model in which you pay your providers directly — through a membership or visit fee — rather than routing everything through insurance. Removing the insurance middleman frees us from billing constraints, prior authorization delays, and time limits that compromise care.
The result is a fundamentally different relationship: more time with your providers, more responsive access, and care that's truly built around you.
Membership at Prevent & Sustain provides ongoing access to your providers, priority scheduling, and continuity of care at a predictable monthly cost. Specific membership tiers and inclusions are outlined on our Memberships page.
Some services — such as IV therapy, injections, advanced labs, and certain procedures — are available à la carte with member discounts applied.
No. We offer individual visit-based appointments for patients who prefer not to commit to a monthly membership. Payment plans are also available. We believe access to thoughtful, personalized care shouldn't require a long-term contract if that's not the right fit for you.
Your initial consultation is scheduled for a full 60 minutes — enough time to take a thorough history, review your labs and concerns, and start building a real picture of your health. Follow-up visits are tailored in length to what you need; we don't rush you out the door to stay on schedule.
No. Direct Primary Care is not health insurance and does not replace it. You still need insurance (or another coverage option) for hospitalizations, specialist visits, imaging, and emergency care. DPC works alongside your insurance by handling a broad range of primary, preventive, and chronic care needs directly — often more affordably and with far greater access.
Absolutely. We regularly coordinate with specialists and other providers. We can facilitate referrals, review specialist notes with you, and help you navigate complex diagnoses — functioning as your medical home base and advocate across your entire care picture.
Insurance & Payment
We are currently in the process of credentialing with all major commercial insurance providers. We will update patients as that process is completed.
In the meantime, we operate on a transparent cash-pay basis — no surprise bills, no coding games, no claim denials that delay your care.
We do not bill Medicare or Medicaid for primary care or most services. However, patients covered by Medicare or Medicaid are welcome to participate in our weight loss therapy program — those services are available to everyone, regardless of insurance status.
Yes. Many patients use their Health Savings Account (HSA) or Flexible Spending Account (FSA) to cover visit fees, membership costs, and other eligible services. We recommend confirming with your plan administrator which services qualify under your specific account.
We accept major credit and debit cards, HSA/FSA cards, and cash. Payment plans are also available — visit our Payment Plans page for details, or ask us at your first appointment.
In most cases, yes. When we order labs or write prescriptions, you can use your insurance to fill those through your pharmacy or lab of choice — just as you would with any provider. We also offer deeply discounted in-house lab options for patients who prefer a straightforward cash price.
Weight Loss Therapy
Our approach to weight loss is medical and individualized — not a one-size-fits-all diet plan. We start by understanding why the weight is there: hormonal imbalances, thyroid dysfunction, insulin resistance, metabolic disruption, or other underlying factors.
Treatment may include metabolic and hormonal testing, medical weight loss protocols, GLP-1 receptor agonist therapy (such as semaglutide) when appropriate, nutritional guidance, and ongoing support and accountability.
Yes, when clinically appropriate. GLP-1 receptor agonists are a powerful tool for the right patients, but they work best as part of a broader plan that addresses the root metabolic picture. We evaluate each patient individually and will discuss options, benefits, and considerations with you during your visit.
Yes. Although we do not bill Medicare or Medicaid for primary care services, patients on Medicare or Medicaid are fully welcome to participate in our weight loss therapy program. Everyone deserves access to evidence-based weight management support, and we don't want insurance status to be a barrier.
Commercial programs often treat weight as a willpower problem. We treat it as a medical one. By evaluating your hormones, thyroid function, insulin sensitivity, and inflammatory markers, we can identify and address the physiological barriers that make weight loss difficult — and build a sustainable plan around your actual biology.

Yes. We provide comprehensive hormone optimization for both men and women, including bioidentical hormone replacement therapy (BHRT), pellet therapy, and testosterone optimization. Treatment is guided by advanced lab panels and ongoing monitoring — not a one-size protocol.
Peptides are short chains of amino acids that signal specific biological processes — tissue repair, immune modulation, metabolism, cognitive function, and more. We use targeted peptide protocols as part of individualized treatment plans. Options may include growth hormone-releasing peptides, immune-modulating peptides, gut healing peptides, and others depending on your goals and evaluation.
We go well beyond standard panels. Our advanced testing capabilities include comprehensive metabolic and hormone panels, thyroid deep-dives (TSH, Free T3, Free T4, antibodies), inflammatory and autoimmune markers, micronutrient and vitamin analysis, insulin resistance and metabolic syndrome screening, renal and cardiovascular risk panels, salivary cortisol testing (which captures 24-hour patterns far better than a single blood draw), and gut microbiome testing.
Yes. Our IV therapy menu includes Myers' Cocktail, Tri-Immune Boost, Glutathione, NAD+, and Circulate (a blood flow and microcirculation blend) — all with 1 liter of fluids. Absolutely. We provide comprehensive care for both men and women across all of our service areas — hormone optimization, sexual health, metabolic health, and beyond. We also have particular depth in women's health, including perimenopause and menopause care and pelvic floor conditions that are frequently undertreated.

Your initial consultation is 60 minutes. We'll take a thorough history — your symptoms, your story, your goals — and review any prior labs or records you bring. We may order advanced testing at or after this visit. By the end, you'll have a clear picture of next steps and a plan that actually addresses what's going on.
Expect to feel heard. That's the whole point.
Please bring any recent lab work or records from other providers, a current list of medications and supplements, your insurance card (even if we aren't billing it yet, it's helpful for labs and referrals), and any notes about symptoms or concerns you've been wanting to address. The more context you bring, the more productive we can make your time.
